OFFICE POLICIES Our office staff will be happy to assist you, answer your questions and make every effort to accommodate your schedule. CANCELLATIONS / NO-SHOWS
Please remember that we reserve the appointment time especially for you. If you are unable to keep the appointment, please kindly provide us with 24 hours of notice so that we can make this time available to another patient.
Our office also charges a $25 fee for a No-Show to an appointment.
FINANCIAL POLICY
As part of our new patient paperwork we require you to fill out our financial policy. We do request that a credit card number is given at your first appointment. The policy is scanned directly into your account and shredded on the day of your appointment, or you may elect to take your signed copy home with you.
We are contracted with most insurance companies and do submit to your insurance. Your credit card is only utilized if the patient responsibility portion of your charges, including charges applied to your deductible and/or coinsurance, are not paid in full within [90] days. Our billing department sends patient statements directly to the address you have given our office as your primary residence.
